By signing this form, you are giving your child(ren) permission to participate in programs offered for their age group and to travel off site to the spray park, the park, the soccer field outside, and other nearby areas. Information on field trips is available on our SRC Calendar Page.

While the Stettler Public Library takes reasonable care to provide safe programs and supervision, participation in activities may involve normal risks associated with children’s play and recreation. By signing below, you acknowledge these risks and agree not to hold the Stettler Public Library liable for accidental injuries or incidents that are not the result of negligence.

The program is drop-in, but we do ask that you complete the following information in case we need to contact you.
